Patient Financial Responsibilities

By agreeing to this Financial Responsibility Agreement, you acknowledge and accept the following:

  1. Payment for Services:
    • You are responsible for all charges associated with the healthcare services you receive at Ababe Health Care Center, including office visits, procedures, tests, and treatments.
    • Payment is due at the time of service unless other arrangements have been made in advance.
  2. Insurance Coverage:
    • If you have health insurance, you are responsible for providing accurate and up-to-date insurance information at the time of service.
    • You are responsible for any co-pays, deductibles, co-insurance, or non-covered services as outlined by your insurance plan.
    • If your insurance company denies a claim or does not pay within a reasonable time, you will be responsible for the outstanding balance.
  3. Self-Pay Patients:
    • If you do not have health insurance, you are considered a self-pay patient and are responsible for the full cost of services rendered.
    • We offer payment plans or discounts for self-pay patients, subject to approval. Please contact our billing department for more information.
  4. Missed Appointments:
    • If you fail to cancel or reschedule an appointment at least 24 hours in advance, you may be charged a missed appointment fee.
  5. Collections:
    • If your account becomes delinquent, it may be referred to a collections agency. You will be responsible for any additional fees associated with the collections process.

Billing and Payment Information

  • Statements: We will send you a statement for any balances due. Please review your statement carefully and contact our billing department with any questions.
  • Payment Methods: We accept cash, credit/debit cards, and checks. Payment can be made in person, online, or by mail.
